Immunisation is a simple, safe and effective way of protecting people against harmful diseases before they come into contact with them. We undertake the school based immunisation programs on behalf of the State Government's Department of Health (DOH).
Council maintains a record of previous immunisations which it has conducted. If you need information on what vaccines you or your child has received, please complete the below form.

Parents please note: Council is unable to release information to you if your child is over 18 years, however you may complete the request for your own records as an adult.
